Over 200 Years of Combined Experience

Four Corners facility was originally built in the 1960's. It has undergone many changes throughout the years. Probably one of the largest and most significant was the addition of our skilled unit in 1993. This expansion added an additional 40 beds and an in-patient and outpatient rehabilitation gym that is still in use to this day. In the early 2000's, Sava Senior Care purchased our facility. This company continues to operate our facility as well as 198 other facilities across the United States. In 2013, our facility again underwent significant changes with the designation and branding of our rehabilitation unit, Junction Creek Rehab. This unit offers private rooms, a full therapy department, specially trained nursing staff and a true focus on rehabilitative care. Our long-term care units in the facility have also undergone significant change with the push for culture change and the creation of a home-like environment. We have also enhanced our dining programs, activities programs and our many outdoor recreational areas.

Our Management Team

Michelle Greer, RN, NHA

Michelle started her career in long term care at a young age. She began working at Four Corners Health Care when she was only 15 and her passion for the industry grew from that point. She pursued a career in nursing and despite many nursing opportunities and challenges, always seemed to be drawn back to long-term care. She most currently served as the Director of Nursing at Four Corners Health Care from 2008-2013. In 2013, the opportunity to become the administrator presented itself and she decided to obtain her Nursing Home Administrator license and transition into the role of administrator at the facility.

Kerby Jo Rogers, Admissions Director

Kerby Jo volunteered at Four Corners Health Care in high school and loved the experience and interaction with the population we serve. When a position became open in 2010, she felt the calling and decided to pursue employment with the facility. She began her career at Four Corners as the receptionist, but quickly transitioned into the role of Customer Service Director. In December she was offered the opportunity to continue her career growth as the Admissions Director which she gladly accepted. Her passion for excellence in service and care has only continued to grow and is serving her well in her current position.

Erica Beighley SSD

Erica moved to Durango in 2011 and began serving our community and meeting needs through the community shelter. She became a part of the team at Four Corners Health Care in 2012 as the Social Services Director. Her vast knowledge of the geriatric population and of the many needs in our community has served her well in her role. She is a supporter of culture change within the industry and has been a driving force of the process within the facility. She is a valuable resource and advocate for our patients and residents at the facility.

Our Therapy Team

Cheri Kelly - MA CCC-SLP, Rehabilitation Program Manager

After serving the Durango community as a Speech-Language Pathologist at Junction Creek Rehabilitation and Four Corners Health Care Center, Cheri was offered the position of Rehabilitation Program Manager. Cheri is looking forward to helping people in our community obtain and maintain the highest quality of life and functional abilities possible by facilitating their rehabilitation care. Cheri received her undergraduate degree in Organizational Development from the University of Michigan and her Masters in Speech-Language Pathology from Northwestern University.
